Project Assessment

The Project Assessment is the second stage in the Food Distribution Assistance Program application process for organizations seeking funding through the Food Bank Partner Agency (Potential New and Existing) funding opportunity. 

ONLY ORGANIZATIONS THAT HAVE MET THE REQUIREMENTS OF THE PRE-APPLICATION PROCESS ARE ELIGIBLE TO SUBMIT A PROJECT ASSESSMENT.

Project Assessments will be accepted beginning Friday, February 17 at NOON and are due by Friday, March 3 at NOON. You will receive an automated email confirming submission of your Project Assessment. If you do not receive an email confirmation, please email FDAP@goldenleaf.org.

Organizations are encouraged to download, review, and submit the required documents for the Project Assessment as soon as possible. Project Assessments that include incorrect or incomplete documents as of the submission deadline will not be eligible for review.

Important Reminders

Download the Project Assessment Questionnaire from the resources page. Save the document as a Word or PDF file. 

The Project Assessment upload form cannot be started and saved. You must complete the form in one sitting. Please gather all necessary documents prior to beginning to complete the form. The Project Assessment upload form must be totally complete, including all required attachments, to be submitted. The completed Project Assessment Questionnaire is one of the required attachments.

Please ensure all information is entered correctly, especially the email address of the project contact.

This is not a grant application, but a required step in order to be potentially eligible to submit a grant application later.

For the purposes of the Project Assessment upload form and the Project Assessment Questionnaire, the county or counties served should only include those where food distribution activities will physically occur. This will not reflect those individuals or households from neighboring counties that may benefit; that information will be gathered later in the application process.

Organizations must submit a separate Project Assessment for each project for which they intend to submit an application. Projects must be distinguishable by location to be considered separate projects.
